Specific Activities
- Conducts eligibility screenings and intakes for identified youth
- Conducts quarterly outreach to school districts, organizations that serve at-risk youth, law enforcement, and other referral sources
- Collaborates with Bridgeway Clinicians, SUD Counselors, PCPs, Mental Health Registered Nurse, and other individuals in the client’s care team to develop a comprehensive, holistic case management plan and goals
- Co-facilitates evidence-based treatment groups with Bridgeway Clinicians
- Maintains a minimum 30% productivity level
- Completes other duties as assigned
Qualifications/Requirements
- Lived experience and/or education from related field, with behavioral health, juvenile justice, and substance abuse disorders between the ages of 12-21 preferred.
- Experience of community-based social service or case management experience is a plus.
- Must possess the ability to make sound decisions independently.
Additional Requirement
- Must have a valid driver’s license and reliable transportation with a good driving record.
- Willing to use own, properly insured, reliable vehicle for the job (mileage reimbursed).
- Employment subject to clearances, including background finger printing checks, driving record, tuberculosis testing, and drug testing.

$18-$19.60
Salary will commensurate with experience and education. Differential pay offered for bilingual skills. EEO
