Connections Community Counseling

Homeless youth are a historically underserved community who present with an elevated risk of mental health disorders, including anxiety, depression, PTSD, and suicide. Providing ongoing therapeutic support and community resources will allow youth to develop the skills needed to cope with their current challenges and become independent and successful adults.
PROGRAM SUMMARY:
Connections Community Counseling is a mental health program for youth, up to age 21, who are homeless, runaway, or residing in inadequate or non-permanent living situations (couch surfing, transitional housing, multifamily living, threatened placement, etc.). Services are provided to youth and their adult support persons at our San Diego clinic and at various locations across the county to ensure accessibility. Master's level clinicians will provide case management services and ongoing individual and family therapy while youth partners with lived experience will work alongside clients to provide psychoeducation and skill-building
PROGRAM SERVICES:
- Full behavioral health assessment
- Individual and family therapy
- Referral to psychiatry and case management
- Linkage to community services

OUR IMPACT:
Connections Community Counseling's goal is to support the health and wellbeing of homeless youth by addressing mental health needs and providing community resources. Proposed outcomes include:
- Decrease in mental health symptomology
- Increased connection to community resources
- Decrease in family conflict and stress
- Maintain safety and stable living environment
